Patient Pricing:

• Level 1 ($99): one simple problem
• Level 2 ($129): two simple problems or one complicated problem
• Level 3 ($149): three simple problems or one simple and one complicated problems or wellness exam or one procedure
• Level 4 ($189): multiple complicated problems/procedures
• Hourly Counseling ($110)

Please note: With cash pay healthcare the level of service is determined by the provider and is up to their discretion.  Any labs or additional tests not included. Pricing excludes any new patient fees.
